Oesophagus letsel

Verdenking oesophagus letsel

– Bij pneumo/haemato thorax links zonder ribfractuur;

– bij inwerkend geweld caudaal over het sternum of de regio epigastrio;

– bij aanwezigheid van veel pijn of verschijnselen shock zonder uitwendige tekenen ernstig letsel;

– bij aanwezigheid pneumomediastinum;

– bij aanwezigheid voedselpartikels in vocht thoraxdrain.


Eerste opvang
Waarschuwen na melding CPA of bij binnenkomst patiënt 

CAVE
– Respiratoire problemen
– Cardiale problemen
– Circulatoire problemen

Diagnostiek

– Auscultatie ademhaling, auscultatie hart
– Saturatie, pols, tensie, ECG
– X Thorax (let op: bloeding, pneumothorax, spannings-pneumothorax, rib #, mediastinum, sternum) (na 1 uur herhalen)
– Bij verdenking op aortaletsel: CT angiografie (chirurg overleg met radioloog)
– Bloedafname art. bloedgas Bloedgroep, Hb., Ht., leuco’s, CPK-Mb, Troponine INR

– Contrastfoto’s (gastrografine),CT-scan + contrast

– Endoscopie.

 

Kinderen

Bij kleine kinderen geen gastrografine gebruiken i.v.m. de hoge osmolariteit doch Omnipaque of gelijkwaardig middel met lagere osmolariteit, i.v.m. mogelijke aspiratie en de ernstige schade door hoog-osmolaire vloeistoffen.        

 

Behandeling   

Indien spanningspneumothorax

•      Spoedthoraxdrainage m.b.v. naaldpunctie, 

•      2e intercostaalruimte midclaviculair 16 G naald.

•      In een latere fase definitieve thoraxdrainage op de SEH

 

Zuurstoftherapie

• 100% zuurstof via non-rebreathing masker 

• Bij patiënt bekend met COPD: 1 liter O2/ min.  

• Low energy trauma: 2 liter O2 min.

 

Longcontusie, haemoptoe

Longarts in consult

 

Indien harttamponade

Pericardpunctie

 

Indien haematothorax/ pneumothorax > 1500 ml bloed  óf 200 ml bloedverlies per  uur   

thoracotomie

 

ECG afwijkingen

CPK-Mb/ Troponine verhoging

Cardioloog in consult

 

Nabehandeling

Afhankelijk letsel

Controleschema p.k.
Indien geen opname indicatie: 1 week

 

Contusio cordis
Een patiënt met een thoraxtrauma bij een frontaal hoogenergetisch ongeval, een trap van een paard of een sportongeluk, waardoor in het myocard bloedingen en/ of oedeem ontstaat. 

Inclusiecriteria stroomdiagram verdenking contusio cordis

  • Gordelhematoom
  • 1 ribfractuur met hematopneumothorax
  • 2-3 ribfracturen of multiple fracturen 1 rib
  • Hemato- en/of pneumothorax
  • Bronchus-trachea-oesophagus laceratie
  • Intrathoracaal vaatletsel 
  • Diafragmaruptuur
  • Sternumfractuur
  • Structureel cardiaal letsel: pericardletsel, klepletsel, wandruptuur, septumruptuur
  • Longcontusie , -laceratie
